With the combination of Torii's Application Catalog and the power of Torii workflows, you can easily create a request application access workflow.
Follow these steps:

Create a request access workflow

Setup request access button from the application catalog

Step 1 - Create a Request Access Workflow
The example workflow below will be triggered when a user requests access to an existing application from the catalog. The requesting user will be asked to fill in a short form and the form submission will be sent to the app owner for approval.
Step 1.1 - Request Access Trigger

Navigate to the workflows page and create a new workflow

On the trigger type, select "Request access"

Step 1.2 - Send Application Form to the App Owner
Add "Send application form" action
In the action email setup, complete the details:

App:
The app that the user requested access to
-
From:
The user that requested the access
-
To:
App owner
-
Fill in the subject line and text
In the action form setup, complete the details:

Form intro: Type the intro message that will appear on the short form

Select form fields: Pick questions you would like to ask the requesting user

Send Form Submissions To:
App Owner


Step 1.3 - Request Approval from the App Owner
Add "Request approval" action
In the action email setup, complete the details:
